Not really have seen it before, the BCG returns just enough to strip the round but wont push it back all the way.
TotT are you using a rifle length buffer and spring? or a carbine length? I have seen it happen quite a bit on 300AAC's and with the really short barreled rifles (7"-10") when people dont get the port size just right. On them the powder doesnt burn completely until its out of the barrel so it doesn't make enough gas inside the barrel. With your combo there could be a few different things going on.
1. carbine length buffer tube, Rifle Spring (Believe it or not, it happens alot especially in the military where M16A4's and M4's mingle. There was a tech sheet on it in Armorers classes. (Not MOS school but AD Armorers school)
2.Out of spec gas hole in the Barrel. way too big or way too small.
