I have encountered a number of extractors with too sharp edges (some ever so slightly burred). This causes the extractor to bite into the case too much on extraction and can result in failures to eject. The easy remedy is kissing the edges of the groove cut into the extractor with a jewelers file to knock off the sharp edge. You should probably also check your ejector button tension as well, but my best bet would be on a sharp extractor being involved if I'm guessing blindly out the gate.
