I'd take a hard long look at the throat. Looks like a burr or something scoring the bullet when the bolt goes into battery.
As far as the upper assy, we can probably rule that out now. Just the Barrel and bullet based on your first target pic.
Now - SST - That is a damn picky bullet as to what barrel it will fire out of. You can have 5 same length barrels by the same MFG, and it might fire good out of 2.
Try a different bullet with CFE under it. You listed `123 ELD, so that is a good one to choose (Typo - SST or ELD?)
8208 is known to spike pressures under pills above 120 rather quickly.
You mentioned "Way too slow" but you did not post any velocity readings. With a 20" 2475 to 2550 should be a good speed for the 123s
