There are some critical dimensions with the carrier that can equal reliability issues, but it seems that most makers have their CNC specs and QC down tight. The hardest thing after that is hard chrome-plating the slot for the bolt. There are a lot of carriers made in Taiwan.
There are also issues from time-to-time with machine shops making the OD of the guide rails larger than spec, so you have sluggish operation & lots of malfs, although the one company that comes to mind seemed to have gotten their act together after that.
