I like that you're checking everything carefully and not rushing into anything. LAST thing you want to do is open the gas port if it's locking back. Gotta be something else.
1. DOES it lock back on your 95gr load? If it does, I don't think it's the gas system. You said the carbon ring from the gas block is centered on the gas port hole, so that's good.
2. You mentioned it could just not be broken in enough with only 100 rounds on it. Go to 200 before get getting serious about altering parts.
3. What I'm really thinking is extractor.
a. Is there too much oil in the extractor claw groove causing the extractor to slip off the rim randomly? Did I read it wrong or did you say it's sometimes leaves a case unextracted in the receiver? If so, that sounds like an extractor slippage, which says oily or not enough tension but it DOES have an O-ring. Hmmm....
b. With a bone dry extractor claw, try with and without the O-ring. (I've only found them necessary on 12" CLGS; others find them not necessary at all experiment with this.
c. Try a completely different bolt and extractor? (If you don't have one, I can send a loaner.)
