Type II bolt spec and questions...

Looking for some more detail on type II bolt specification. I have heard folks say that the spec is 0.136 +0.00/-0.003. Is this true, and where is the data to support? What's interesting is that a type II bolt (which should be .136) may be more like 0.138 or more, and if they use it during chambering to set headspace, it could lead to a condition in which A. it headspace gauges fine and doesn't close on a no go, AND B. leads to unsupported case area if a generous feed cone is present, leading to larger than normal case head swelling and what appears as a belt near the case webbing after sizing. Tell me if I'm off the mark with these thoughts...
