That normally points to a failure node in the brass.
You normally dont see the extractor survive an overpressure event in the AR-15, and 1-2 bolt lugs often will shear off.
With the bolt and extractor still intact, I think you just had a bad piece of brass with stretching in that circumferential area around the case web.
Do you have any pictures of your once-fired brass that havent been re-sized?
Kind of what I was thinking at first...Bad case.
No I do not, I cleaned and loaded all my empties that day.
