This is why I always use factory ammo with a new rifle or pistol. If it is an ammo problem you may have some recourse with the manufacturer.
That looks like a hot load to me, total case head separation. You got very lucky your bolt and chamber held it together as well as it did and your upper and lower did not turn into little pieces of shrapnel injuring you. Glad your OK.
