I have seen this as well. If you try to place one of the O or D rings as an "enhancement", it often produces the results you experienced. There's a lot to be said for a quality, standard Milspec spring and buffer insert, balanced with a quality ejector spring and ejector made to the appropriate dimensions for the caliber.
