I tracked my stovepipes back to an over powerful extractor spring. The spring would press the case onto the opposite side of the bolt with such force that the ejector couldn't push it off quickly enough during cycling. Once I swapped out the extractor spring for one without so much power, everything started working just like it should.

I love cheap and easy fixes. It is just another thing to consider when you start sorting through all the suggestions you receive here.
