Something is really gouging up your brass.
I've never seen that kind of damage on the brass.
Check the chamber for being clear of debris or a defect.
Clean it thoroughly with a brush, and clean the bore.
Make sure it is dry, then cycle another round and see what it looks like (Pointed in a Safe direction).
