Having watched his pistol appear in his hand almost as if it were a magic trick, I have very little concern for his anatomy. It was a great concern of mine as well, but having tried the carry position, I realize that my Dunlops disease is the limiting factor, and I'm making a lot of effort to get rid of it. Properly positioned, it points straight down the anterior leg, much like hip or behind the hip points down those areas. But the draw from the appendix position is incredibly fast in his case, because you can pull the shirt straight up with the left hand without having to reach across. In his case, with his practice level, he is faster than 95+% of people open carrying, I am 100% sure.
