I was using Hornady Brass, WSR primers, and I was having problems with FTF... I am going to assume that I was not pushing the shoulder back far enough and it was not chambering as there was no indentation on the primer, so the pin had not made contact with the primer. I had a hell of a time trying to pull the charging handle back to extract them, and manually fed them in to try again. Of the 5 I had, one of them fired with the slide of the bolt forward... Scared the crap out of me and the guys next to me were glad that I had practiced range safety and kept the muzzle downrange... That was only 1 instance, and I need to re-adjust my dies to push the shoulder back a little bit. I kept them back to try to alleviate working the brass as much as I could... My fault there.

I think you nailed it on the head, many of us had this problem, get the Sheridan case gauge.
